The form does request "SIGNATURE FULL NAME" for the signature line.
You sometimes see the middle initial on some of his credit cards.

Don't think it's misspelled. In that doc he filled out most of it, and looks like he did it in a hurry (boring red tape). The second 'e' is mushed into the 'y' and I'm sure having to print this stuff out was irritating. He was a man in a hurry to get his 9mm Walther "James Bond" type handgun.

That's his printing, not handwriting, silly.
Elvis' cursive work was decent, though. Just check those army letters to Anita Wood and Alan Fortas.
He was in less of a hurry, I think.
